Part A is for the basic details of your school. Part B allows you to order up to twelve separate collections on the same form. (Special Schools – please enter the Year Group level at which you are working, as well as actual Year Group.)
'Submit Form' will send to SLS the details you have entered.  'Reset Form' will clear any details you have entered without sending them.
