Thank you for accessing the SLS Statement of Requirements (SoR) page. From here you can complete an SoR on line.

It assists us greatly to plan our resources if you indicate on the SoR form the SLS services that your school is likely to require during the year April to March.

However, you may still request any of our services even if you do not complete an SoR.

If you know your requirements - go to the appropriate SoR form by clicking the appropriate link on the left i.e. 'Primary SoR form' or 'Secondary SoR form'. (Nurseries and special schools - please use the Primary form).

For a description of the options that appear on the SoR form... click the appropriate 'Options' link on the left.

Don't hesitate to contact SLS if you need to discuss your requirements - use the 'About SLS' tab above and click 'SLS Contact Details'.
